We recommend that you deliver one original to your attending physician for his/her file and that you keep one original Health Care Proxy in an accessible location, should it be necessary to find it without your assistance. DO NOT PLACE YOUR HEALTH CARE PROXY IN A LOCKED SAFE DEPOSIT BOX EITHER AT HOME OR THE BANK.
A copy of your Health Care Proxy will be retained in our office. If a hospital, doctor or other health care provider requests that you sign his, her or its Health Care form, I will be happy to review the form for you. As long as you have the same persons designated as health care agents, however, in the same order as you have on your present Health Care Proxy, you may be comfortable signing the requested form.
If in the future you spend an extended period of time in another state (e.g., Florida, Arizona), you should execute a Health Care Proxy, or some similar surrogate medical decision-making instrument, for that state.
